Is there any time you have given thought of taking him to your vet? The dog needs a complete health check. Your vet will guide you on what can be done for your dog.
Take him to a canine orthopedic vet. Your vet can refer you to one. The specialist can replace the hips.
I would take him to the vet asap. Falling is not good either. Hate to say it but it may be time. My shepherd had something similar and when he started falling was when I took him for his last visit.
My vet who handles all of the Phila K-9s said when these dogs can no longer run, they are very very unhappy.
When a working, sporting dog like a shepherd can no longer run they will become mentally bored frustrated etc. Also if he is falling he could hurt him self seriously. Take him to the vet. Sorry
You need to consult with your veterinarian, and probably an orthopedic specialist, to discuss your options. Hip replacement surgery is a possibility, although it is expensive. If that is not an option, there are wheelchairs available for dogs. Euthanasia is an option as well if yor vet does not think the dog would benefit from surgery and if sufficient pain relief is not possible.
As for pooing in the house, again, you need to check with your vet. It could be that his mobility problems are keeping him from asking to go out, and it may be that he is in enough discomfort that he won’t get up to go until the last possible second. However, it is also possible that he is ill and the problem is unrelated to the hip dysplasia.

Change dog food to rice get it away from corn formulas that way the dog only has a bowel movement once or twice a day..I tried many formulas like shark marel(which was hard on stomach, also kelp which was alright in small portions(horses are given this for their hips..I finally gave in and put my dog down because I could only prolong her misery so long…I had to think or her not me good luck
Hip dysplasia is a really heavy arthritis kind of thing, not an illness, and there’s a lot you can do to make it easier on the dog.
Take him in for a checkup and talk to your vet about dysplasia and something called glucosamine chronditin. He’s not ‘old’ he has at least 5 more years in him. You need to start helping him deal with his problem now, not later when things are even worse.
